This membership is designed for clients who want to maintain a smooth, refreshed upper face year-round with predictable monthly pricing. Must commit to an appointment every 3 months.

Important Botox Membership Terms
- Membership funds are for Botox only
- Membership funds are non-transferable
- Membership benefits and discounts apply to active members only
Terms and Conditions:
- Membership pricing, promotions, and offers are non-transferable and for member use only.
- Membership funds may ONLY BE USED FOR SERVICES.
- Membership pricing cannot be combined with any specials or flash sales.
- Monthly dues cannot be paid for by vendor loyalty programs.
- Membership fee must be paid by accepted credit cards. PAYMENTS ARE NON-REFUNDABLE.
- Any unused credit may rollover to the next month for a total of 90 days in which you must use your funds or lose them.
- If a membership account is past due for more than 10 days, there will be a $50 re-enrollment fee applied. This includes any member whose credit card on file has expired or has insufficient funds and the correct credit card is not given within the 10 days.
- In order to cancel after the minimum term of 12 months has been completed, a 30-day written cancellation notice must be given. All memberships will be auto renewed if not cancelled.
- Memberships that are cancelled within the first 6 months will be charged a $250 fee.
- Unused funds upon early cancellation (before 12-months) will be forfeited immediately.
- Unused funds upon cancellation at end of 12-month obligation must be used within 30 days of cancellation date or funds are forfeited.
